About the Role
We are seeking experienced Support Workers to join our team in Northbridge. You will support NDIS participants with psychosocial disabilities and complex mental health needs to live safely and independently in a structured home setting.
Key Responsibilities
Deliver person-centred support aligned with NDIS plans
Assist with personal care, daily living, and household tasks
Support participants with mental health conditions and behaviours of concern
Implement Positive Behaviour Support Plans (PBSP)
Administer medication (where trained)
Maintain accurate progress notes and incident reports
Promote community participation and independence
Respond to incidents and escalate risks appropriately
Essential Criteria
Minimum 1–2 years’ experience in mental health / psychosocial disability support
Experience in SIL,
group homes, or residential settings
Certificate III or IV in Individual Support, Mental Health, or equivalent
Current NDIS Worker Screening Check
Current First Aid & CPR
Driver’s licence and reliable vehicle
Solid communication and documentation skills
Highly Regarded
Experience with challenging behaviours and crisis management
Knowledge of NDIS compliance and reporting requirements
Medication competency and restrictive practices experience
