Livable seeks an experienced Care Partner - Support at Home/CHSP to join our Aged Care team in Grafton and/or Coffs Harbour, delivering funded aged care services 38 hours per week. You will coordinate services reflecting participants' needs and collaborate with families and supporters.
Key requirements include Cert III in Aged Care, 3 years' experience, exceptional communication, and ability to work with diverse participants. NSDS orientation and checks will be completed as part of recruitment.
