About the role:
The Community Home Care Clinical Case Manager will help to ensure high standards of the services are person centered, responsive and support clients to live as well and independently as is possible in their own home and community based across the Rockhampton region.
To help achieve this purpose, the Clinical Case Manager will provide effective support and leadership to the team and focused on delivering high-quality, person-centred care and strong clinical governance across the region.
Duties:
Lead the Clinical team to manage CHSP, HACCandHCP programs to ensure appropriate services are delivered that meet clientneeds.
Ensure services meet Aged Care Quality Standards, legislative requirements and organisational policies
Lead clinical risk management, including incident oversight, investigation and continuous improvement actions
Monitor clinical governance frameworks and implement strategies to enhance safety and quality of care
Conduct audits, reviews and reporting to support compliance and accreditation
Develop a continuousimprovement culture and mentor Registered Nurse's in the resolution of client careissues and problems.
Oversee practice in clinical assessment, care planning and ongoing reviews aligned to client goals and preferences
Support best practice clinical interventions including chronic disease management, wound care and palliative care
Identify and resolve clinical issues asthey arise.
Skills and experience:
Current AHPRA registration as a Registered Nurse
Demonstrated leadership experience in community or aged care services
Strong understanding of Aged Care Quality Standards and regulatory requirements
Experience in clinical governance, risk management and quality improvement
