Rochester and Elmore District Health Service is seeking a dedicated Lifestyle Team Leader to oversee lifestyle, wellbeing and social support programmes across Yalukang Aged Care. You will collaborate with residents, families and staff to design person-centred activities that promote independence, identity and community engagement.
In this role you will lead a small team of Lifestyle Assistants, ensure program delivery aligns with Aged Care Quality Standards and contribute to service development
