passionate about supporting patients with timely and safe hospital discharge, we want to hear from you.
As a valued member of our team, you will collaborate with a multidisciplinary team to enhance patient flow, deliver optimal care, and reduce hospital re-admissions. This role is your chance to play an essential part in ensuring patients receive the right care, at the right time, in the right place.
What you bring
Minimum of 3 years' experience in your health profession
Creativity, flexibility, and the ability to adapt to organisational changes
Strong time management, organisation, and problem-solving skills
The ability to work independently while contributing to a multidisciplinary team
Excellent interpersonal and communication skills
Experience in subacute and community settings (desirable but not essential)
