You'll act as a vital link between primary care providers, RASA, and specialist services—strengthening referral pathways and promoting integrated, holistic care for those affected by FDSV.
You will work in partnership with local primary health care services (PCHS) in the Northern Metropolitan region and you'll provide short-term, trauma-informed case management and service navigation.
Your role includes conducting risk and safety assessments, developing safety plans, and coordinating referrals to ensure timely, culturally secure, and client-centered support.
