About the Role
You will work as part of an integrated multidisciplinary Allied Health team delivering person-centred services across aged care, restorative and community environments.
The role may include:
Functional assessments (Personal Care, Kitchen and Laundry Assessments, etc)
Equipment prescription and education
Environmental assessments (Home Modifications)
Reablement and restorative interventions
Falls risk assessment and falls prevention strategies
Cognition assessment and dementia support strategies
Supporting older people to maintain/improve independence, dignity and quality of life
About You
We are looking for people who have:
Work experience in community aged care, experience working with TCP and SaH clients is desirable but not essential
Passionate about older people and quality care
Practical, collaborative and compassionate
Strong communicators
Open to innovation and service improvement
Registered with AHPRA
