Location: Aveo Northern Beaches Communities – Avalon Beach, East Lindfield, Mosman and St. Ives

Reports to: Regional Home Care Manager

The Opportunity

Aveo Care at Home delivers tailored care through the government-funded Support at Home program, helping older Australians live independently in their own homes. As aCare Partner (Clinical), you will manage a caseload of consumers, providing support and guidance to access services, and promoting Aveo as a home care provider.

You will spend your day in residents’ homes to develop personalised plans, coordinate services, and ensure residents receive the ongoing support they need.

Your Impact

- Meet with prospective Aveoresidents and their families to understand and assess their needs
- Provide tailoredinformation on care services and funding options available
- Establish home care plans, ensuring supports are in place as required
- Prepare and maintainrecords, documents and reports as required
- Deliver excellent customer service to residents, families, staff, management, visitors and external health professionals
- Liaise effectively with external service providers who provide care within the community
- Promote Aveo Care at Home services by attending community events,



assisting with the transfer of home care packages and providing support throughout the My Aged Care journey

What you’ll bring

- Current Registered Nurse AHPRA registration
- Prior experience in the home care or community care industry (e.g. Community RN, Case Manager, Care Coordinator and/or exposure to My Aged Care or ACAT)
- Understanding of the Support at Home program (or previous HCP knowledge).
- Excellent customer service, technology and communication skills
- Self-motivated individualwith a practical and skilled approach to case management
- Drivers Licenseand two (2)COVIDVaccinations (required)
