RFM Services in Victoria is seeking a dedicated Disability Support Worker for a full time role (40 hours per week).
You will provide personal care support to promote health and wellbeing and help participants attend appointments and participate in social and leisure activities.Working closely with families, specialists and other stakeholders, you will follow individual support plans and document progress to ensure high-quality, protected services.
A VIC driver's license is required.
