We are seeking a motivated and compassionateEnrolled Endorsed Nurse (EEN)to join our Community Nursing team.
In this role, you will provide comprehensive nursing care toHome Care Package (HCP) recipientsin their own homes.
You will work under the guidance of a Registered Nurse, delivering clinical services and supporting clients to manage their health and wellbeing safely at home.
Key responsibilities
Deliver in-home nursing care including wound care, medication administration, continence care, and chronic disease management.
Complete initial and ongoingclinical assessmentsandregular care reviewsto ensure care plans remain responsive to clients' changing needs.




Conductrisk assessmentsto identify and mitigate potential hazards in the home setting.
Performpost-hospital discharge visitsandpost-falls assessmentsto support recovery and reduce the risk of readmission.
Monitor and report changes in clients' health status to the supervising Registered Nurse.
Educate and support clients and their families in managing health conditions and following care plans.
Maintain accurate and timely clinical documentation and care notes.
Work collaboratively with the broader Home Care team to provide coordinated and holistic care.
Ensure compliance with all clinical standards, safety regulations, and aged care guidelines.
