Grampians Community Health invites applications for a Homelessness Case Manager in Victoria.
Based in Stawell or Ararat, this permanent, full time role offers 38 hours per week to support people who are homeless or at risk, aiming for sustainable housing and improved wellbeing.
You will provide trauma-informed, person-centred case management, applying assessment, planning, coordination and direct intervention within SHS requirements and Housing First principles.
