West Gippsland Healthcare Group in Gippsland VIC is seeking a District Nurse to join the District Nursing Team on a permanent part time basis (up to 128 hours per fortnight). You will deliver autonomous, evidence-based care in clients' homes, with opportunities across District Nursing, Hospital in the Home, and Palliative Care.

You will work with multidisciplinary teams, conduct assessments, plan and evaluate care, and support self-management to promote safety and independence.
