Youth Peer Worker – Pathways Care Team

The Youth Peer Worker will play a crucial role in enhancing the care and assistance provided to young people by offering a unique perspective based on personal lived experiences.

About Us

Parkville Youth Mental Health and Wellbeing Service (PYMHWS) is Victoria’s newest public mental health service dedicated to the care of young people aged 12 to 25 experiencing complex mental health challenges.

PYMHWS is leading a bold and values-driven transformation in youth mental health care across Melbourne’s north and west. The Department

Pathways Care Team (Pathways) is a new program being established as part of the expansion of our Continuing Care Teams (CCT).



Pathways is designed for young people who present with tertiary level mental health needs, but where specialist early intervention programs are not yet indicated or need for this cannot be determined at intake.

Our CCT model delivers ongoing, community‑based care for young people experiencing severe and/or complex mental ill health, including mood, personality, substance use, eating and neuro‑developmental disorders. Pathways will strengthen this model by offering case management with a strong emphasis on care coordination, collaborative care, and engagement to support assessment and brief intervention.

The Role

We are excited to welcome an enthusiastic, passionate and experienced Youth Peer Worker to our diverse team. The role focuses on providing emotional and social support through sharing common experiences with young people, developing relationships that foster hope and optimism, and providing a lived experience perspective within the multidisciplinary team.

Key Responsibilities

- Share lived experience and provide emotional and social support to young people.
- Build and maintain relationships that foster hope and optimism.
- Advocate for young people and connect them to resources and community services.
- Navigate services and support young people in accessing care.
- Provide peer‑to‑peer support as part of the multidisciplinary team.

About You: Essential





- Personal lived experience of mental health challenges and/or mental health service use as a young person.
- Previous experience in a consumer peer support role.
- Understanding of problems young people face when accessing mental health services.
- Ability to work with diverse young people in a compassionate manner and demonstrate sensitivity to diversity.
- Capacity to work independently and autonomously, as well as collaboratively as part of a team.
- Ability to write and keep records.
