About The Role

The Registered Nurse will join our Allied Health team in a community‑based role supporting Home Support Program clients. This position provides direct clinical care while also acting as a clinical leader within the service. You will manage a clinical caseload delivering generalist nursing services including wound care, continence, medication management, diabetes support, and palliative care. In addition, you will play a key role in mentoring less experienced nursing staff and contributing to the ongoing development of clinical practices, procedures, and quality improvements within the team.

Key Responsibilities

- Conduct comprehensive nursing assessments using evidence‑based frameworks
- Develop and review client‑centred clinical goals and care plans
- Deliver nursing interventions including wound care, medication management, pressure care, continence care, and dementia screening
- Educate clients, families, carers, and support workers on care plans and interventions
- Work collaboratively within a multidisciplinary team to support holistic care
- Maintain accurate clinical documentation, reporting, and billing records




- Contribute to service planning, quality improvement, and risk management
- Provide guidance, mentoring, and professional support to nursing colleagues
- Support the development of clinical practices, procedures, and protocols

Requirements

The successful candidate will require National Police clearance prior to appointment (completed by 3Bridges), valid NSW Working With Children Check clearance and current NSW driver’s licence.
