You will build robust client relationships, lead new client onboarding, coordinate personalised care plans, and work closely with care teams to ensure high-quality services are delivered.Key ResponsibilitiesDevelop and maintain trusting relationships with clients and families.Conduct onboarding, care reviews,



and ongoing coordination using a wellness and reablement approach.Create tailored care plans and service agreements.Identify opportunities to expand services through client needs and community connections.Promote services to support growth and enhance outcomes.Work collaboratively with Caregivers and provide constructive feedback.About YouCurrent AHPRA registrationExperience in aged care, case management, or Home Care Packages.Comprahensive knowledge of Support at Home framework.Experience in sales, growth, or business development is desirable.Strong interpersonal and communication skills.Confidence using Microsoft Office and CRM systems.Valid Australian Driver's Licence and willingness to travel across Brisbane North & Moreton Bay.Passion for empowering older Australians.
