What you'll be doing
Getting older isn't the problem.
Finding the right support can be.
As an Assessor - Clinical Aged Care, you'll meet people in their homes, listen to what's important to them, and get a clear picture of how they're managing day-to-day life.
Using the Integrated Assessment Tool (IAT), you'll complete comprehensive assessments that help shape the support people need to keep living life their way.
You'll work closely with family members, carers, and local service providers to make thoughtful recommendations that are practical, safe, and centred around what matters most to each person.
Whether that's helping someone stay connected to their community, access the right services, or feel less overwhelmed by the aged care system, your work will make a real difference.
This is a permanent, full-time role (Monday to Friday) supporting people across the Brisbane South region.
Travel is a regular part of the role as you meet with people and local stakeholders in the community.
When you're not out and about, you'll have the flexibility to complete your administration from home or a co-working space.
You'll thrive in this role if you
Hold a tertiary qualification in a health-related discipline (e.g. nursing, OT, physio, social work) and current AHPRA registration or eligible to be part of a relevant professional association.
Communicate with warmth, professionalism, and clarity, especially when supporting people with complex health needs or sensory and cognitive challenges.
Understand the impact of ageing on individuals and families
Are confident conducting assessments and navigating risk with sensitivity and care
Are comfortable using technology and open to learning new systems
Work well independently but thrive in a collaborative team
Have a valid Driver's Licence, reliable vehicle, and current First Aid, CPR, and National Criminal History Check (or are willing to obtain them), and can meet all necessary checks and declarations required for the role.
