The team:
In our Home Services Team, YellowBridge QLD provides support to a wide range of people in the Toowoomba area, and we are looking for people who have something to bring to our team as Support Worker within our Home Care Program.
The role and you:
In this position you will be required to deliver high standards of support to aged clients in all areas of their lives.
This means supporting people at home and in the community to enable them to remain in their own home and to live rich and fulfilling lives.
The people we support have a wide range of goals and dreams, and we are looking for inspired people who can support them to remain as independent as they can, while living in their own home.
You must be able to work independently and creatively, while understanding the requirements of each person's care.
You must be willing to adapt to new situations quickly and be available for flexible service delivery.
You will be required to deliver high quality WH&S; standards,



participate in career development and professional representation of YellowBridge QLD in the community.
Position Requirements:
High level interpersonal skills, including the ability to work in a multifunctional team and interact with a broad cross-section of people
Experience with maintaining records of service delivery, or ability to acquire the knowledge
Experience in the delivery of aged care services (or similar) would be considered favourably
Minimum of Certificate III in Individual Support (Aged Care) or equivalent
Must have the ability to understand and follow a client's Plan and goals developed by their Care Manager
All applicants are required to have Yellow and Blue cards.
Any further skills or training you hold in the aged care sector would be advantageous.
