Bayside Health is recruiting an experienced Social Worker to join the Alfred Health Trauma Social Work service as the NDIS Social Work Navigator. The role focuses on comprehensive psychosocial assessments, crisis interventions and discharge planning for patients across the trauma stream, including NDIS clients.
Applicants should hold a Bachelor's Degree in Social Work with hospital-based experience, solid assessment and report writing skills, and knowledge of the NDIS framework.
