We're currently seeking an experienced Care Partner- Support at Home/CHSP to join our Aged Care team, working 38 hours per week based in our Grafton and/or Coffs Harbour office.
In this role, you will coordinate the delivery of approved funded aged care services that reflect the individual needs and preferences of participants.

About the Role
As a Care partner you will:
Build rapport with the participant and their family or registered supporter through active engagement
Understand the participant's needs and goals and collaborate to determine how their services will be delivered
Monitor and maintain the participant's budget, ensuring the services delivered are within the budget allocation
Liaise regularly with family and informal carers to ensure the participant's wellbeing and capacity is supported
Develop and maintain professional, customer centred relationships and partnerships with key stakeholders.
About You
You will bring a compassionate approach and a thorough knowledge of program guidelines.
To be considered for this role, you will hold a minimum of:
Minimum Cert III in Aged Care or related discipline and 3 years relevant experience.
You will also bring:
Ability to work with a diverse range of customers and stakeholders, including the aged, people living with a disability, cultural diversity and language other than English, and the ability to adjust ways of working to support best-practice inclusion and accessibility.
Excellent customer service skills with a passion for ensuring the best care outcomes for participants, registered supporters, families and carers.
Exceptional communication skills (clear and concise written and verbal communication).
Valuable computer skills and experience using customer record management systems.
Self-motivated with a strong sense of accountability and the ability to take ownership of tasks and the customer experience.
Working with Children Check
NDIS worker Screening Check
Current Drivers Licence
First Aid Certificate + CPR
You will also be required to complete the NDIS Quality & Safeguards Commission Worker Orientation Module or provide evidence of completion.
