About the Role
We're looking for an experienced Part time Chronic Disease Management (CDM) Nurse to join our Bondi Medical Centre.
In this rewarding role, you'll work closely with GPs, patients, and the wider healthcare team to support the prevention and management of chronic conditions.
You'll play a vital role in empowering patients to take control of their health, improving long-term outcomes, and supporting proactive, preventative healthcare.
Key Responsibilities
Prepare and coordinate GP Management Plans (GPMPs) and Team Care Arrangements (TCAs)
Conduct health assessments and identify patients who may benefit from chronic disease management support
Support and educate patients to better understand and manage their health conditions
Coach patients on sustainable health behaviour changes and lifestyle improvements
Coordinate referrals and ongoing care with multidisciplinary healthcare providers
Partner with GPs to promote preventative healthcare initiatives across the practice
Increase patient engagement in chronic disease management programs
Ensure care plans are reviewed and updated in line with clinical requirements
Complete all administrative tasks associated with CDM appointments accurately and efficiently
About You
Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN)
Demonstrated experience in Chronic Disease Management within a general practice setting
Experience completing health assessments (highly regarded)
Strong organisational and time management skills
Excellent communication and interpersonal skills with a patient-focused approach
A genuine passion for preventative healthcare and improving patient outcomes
The ability to work collaboratively with GPs and the broader healthcare team
