The role combines clinical assessment, health expertise and care coordination to deliver person-centred, evidence-based and culturally safe care.
You will collaborate with clients, families and MDTs, contribute to governance and SIRS-related activities, and support care planning and clinical decision-making in a culturally inclusive team.
