Northern Health is piloting the Older Persons Care Hub to deliver rapid in-home care for older adults at high risk of hospitalisation. The Care Coordinator will be central to care navigation, rapid assessments and coordinating referrals across Northern Health's catchment.
The role requires 5+ years clinical experience, solid care coordination skills and the ability to support older adults with frailty in the community.
