Home Caring seeks an experienced Case Manager to support clients in the Mornington Peninsula and surrounding area. The role is hybrid, combining WFH with community visits and in-office coordination of administration and engagement events.
You will assess and develop person-centred care plans, coordinate services, advocate for clients, and ensure compliance with NDIS and Aged Care standards. RN qualification and/or case management experience preferred.
