About the role:
At Uniting AgeWell, Personal Care Workers co-ordinate personal care for residents in accordance with care plans which meet their individual needs and preferences. Working within a multidisciplinary team, provide effective clinical and emotional support to residents, recognise changes in residents' conditions and escalating observations and concerns.
Deliver high quality,



person centered care to residents and families, including consideration for cultural, spiritual and individual needs
Provide personal care to residents with complex needs such palliative care, dementia support, restorative care and pain management to enhance their wellbeing
Support nursing staff regarding assessment, care planning processes, documentation and evaluation of care outcomes
Monitor the cleanliness, safety, setting and promptly report issues of concern
Permanent Part Time opportunities available for the below shift lines:
Thursday to Saturday 7am - 1.30pm
Thursday to Saturday 7am - 3pm
Sunday to Wednesday 7am - 1.30pm
Sunday to Tuesday 2:45pm - 10pm
Wednesday to Saturday 7am - 3pm
About you:
This is the place for you if you have:
Certificate III in Individual Support, or equivalent (Aged Care)
At least 1 year experience in residential aged care
Experience with dementia care and palliative care support (desired)
Demonstrated abilities to support residents in a fast paced environment with exemplary communication skills in a team and one on one interactions with all levels of care partners

Essentials for working in Aged Care:
A current Working With Children Check card
Current and ongoing flu vaccinations
Ability to pass a national police clearance and NDIS Clearance
