This role focuses on community engagement, coordination and empowerment rather than personal care.
You will play a key part in helping participants navigate services, strengthen their independence and stay socially connected.This is the role for you if you have:A genuine interest in supporting individuals and communities to enhance their wellbeing and reach their potentialStrong interpersonal and communication skillsExcellent organisational and problem-solving abilitiesThe ability to work independently with professionalism and empathyA flexible, proactive, and adaptable approach to a dynamic work environmentExperience in health, welfare, or community services (highly regarded)Qualifications in Community Services, Social Work, Welfare, or related fields (highly regarded)You will be required to:Lead initiatives to obtain grants and funding resources for HeartcareEvaluate local needs and available resources across health, welfare, housing, employment, education, and related servicesCoordinate with community organisations, welfare providers, government agencies, and private sector partners to address community concerns and increase awareness of available support and initiativesIdentify client requirements and design, deliver,
