This is a part time fixed term position 0.4EFT 30.4 hours per fortnight with the Transition of Care Service at the Department of Adolescent Medicine.
You will be responsible for providing consultation, guidance, education and transition planning for RCH patients while participating in combined RCH and adult service transition clinics, research and annual events hosted by the Transition of Care Service.Classification for this position will range from A031-A035.What you'll achieve:Provide care coordination to patients and their families and communicate this plan with all members of their treating teamsEnsure accurate and up-todate patients notes are recorded in the Electronic Medical Record to support the smooth transition to adult health servicesTrack the patient journey through RCH to ensure timely, secure and coordinated transfers to adult health servicesDeliver education and training to RCH staff on transition and support clinicians across the organisation to discuss transition to adult care with patients and familiesBuild and maintain key working relationships with internal departments and external service providersWork in collaboration with other Transition of Care Service team members to ensure excellence in service deliveryYour skills and experienceStrong customer service with the demonstrated ability to engage and maintain key relationships with adolescents and families and ensuring a high level of service to customersDemonstrated experience negotiating with patients with chronic and complex healthcare needs, to obtain information and providing comprehensive support to ensure the best (transition)



outcomes for patients and their familiesAbility to contribute actively and positively as a member of the Transition of Care Service team to ensure ongoing excellence in service delivery and teamworkExcellent communication and interpersonal skills collaborating with a variety of internal and external stakeholdersAbility to balance competing demands while continually contributing to improved service deliveryDemonstrated skills with MS Office applicationsAbout the DepartmentThe Department of Adolescent Medicine provides healthcare and support to adolescents and their families via a range of specialist multidisciplinary services.
