Read on to explore this exciting role and the fantastic perks waiting for you here.About the roleWorking in the Heart Lung Stream, providing social work services to inpatients, families, and carers within Cardiology, Cardio‐Thoracic Surgery, Thoracic Medicine, Thoracic and Cardiac Rehabilitation and Heart and Lung Transplant including psychosocial assessment, risk assessment, counselling, and family supportParticipate in multidisciplinary team meetings to support patient care, discharge planning, and coordination of referrals to community supports, accommodation, and placement servicesAdvocate for patients and carers,



including support with NCAT Guardianship and NDIS applications where requiredContribute to clinical operations including documentation, handovers, statistical reporting, clinical cover, on‐call duties, and resource developmentEngage in professional development activities including supervision, education, research, student training, quality improvement, and mandatory training complianceAbout youHold a degree in Social Work and be eligible for membership with the Australian Association of Social Workers (AASW), with demonstrated clinical experience in a tertiary hospital or health‐related settingStrong clinical skills and knowledge in psychosocial assessment, counselling, therapeutic interventions, case management, advocacy, and complex discharge planningUnderstanding of and practical experience in crisis intervention, short‐term solution‐focused approaches, and grief and loss supportAbility to work effectively within multidisciplinary teams in high‐pressure environments, managing competing demands with robust negotiation and liaison capabilitiesWillingness and ability to participate in the after‐hours on‐call hospital social work crisis service as requiredWhat we can offer youExceptional Rewards: Competitive pay and not‐for‐profit salary packaging to maximise your incomeSeamless NSW Health Mobility:
