Scope is a registered provider under the NDIS and delivers a range of services including supported independent living, short term accommodation (respite), social connections, lifestyle options, therapy and employment services in over 420 service locations across Victoria and NSW.About Your RoleScope has an exciting and unique opportunity for a Service Transition Coordinator to work within our Employment Programs division.In this role, you will coordinate the transition of new clients into Scope services, ensuring a smooth, well-supported onboarding experience that aligns with their individual goals and needs.Key ResponsibilitiesActing as a central point of contact for clients, families and stakeholders during onboardingGathering and documenting client goals, preferences and support requirements in line with funding and compliance standardsCoordinating onboarding activities such as service matching, site visits, planning transitions and funding arrangementsWorking collaboratively with internal teams and external stakeholders to ensure seamless service entryMaintaining accurate records and ensuring documentation meets regulatory and organisational requirementsProviding clear communication regarding services, expectations and funding arrangementsIdentifying opportunities to improve onboarding processes and client experiencesFollowing quality,
