to identify their needs through assessment and person-centred planningOwning the assessment, development and delivery of evidence-based support plans within a complex support environmentDriving meaningful goals and measurable outcomes, using a positive behaviour support framework and a commitment to eliminating restrictive practicesEnsuring plans reflect each participant's goals, strengths and what matters most to themBuilding the capability, confidence and understanding of those supporting participants in their day-to-day livesWorking collaboratively within a multidisciplinary team, including mentoring less experienced practitionersQualificationsTo be successful in this role you will have:A degree qualification as an allied health professionalExperience in report writingStrong organisational skills and the ability to manage a varied caseloadA proactive, self-motivated approach, with a drive to improve outcomes and test new ideasA genuine passion for working in the disability sectorWhat's in it for you?
