Job DescriptionBEHL HOLDINGS PTY LTD As a Family Support Worker at BEHL HOLDINGS PTY LTD, you will provide practical and emotional support to individuals and families, helping them achieve greater independence and wellbeing.
You will assist clients in accessing community services, implementing support plans, promoting positive outcomes, and maintaining accurate records while working collaboratively with families, healthcare professionals, and community organisations to deliver person-centred care.

Coordinate referrals to appropriate health, mental health, housing, disability, education, employment and other community services to ensure clients receive holistic and timely support.
Deliver ongoing family support, parenting guidance and practical assistance to promote independence, resilience, child wellbeing and positive family functioning.
Monitor, review and document clients' progress against established support plans, maintaining accurate case notes and preparing reports in accordance with organisational policies and funding requirements.
Assist young people experiencing social, emotional,



educational or financial difficulties by providing mentoring, advocacy, crisis support and referrals to specialist services where required.
Prepare case reports, funding documentation, service reviews and other administrative records to meet legislative, contractual and organisational requirements.
Liaise with community organisations, welfare agencies, government departments, healthcare professionals and other stakeholders to facilitate integrated service delivery and advocate for clients' needs.
Assess community needs and available resources to connect clients with suitable programs, financial assistance, training opportunities and support services that enhance social inclusion and wellbeing.
Promote awareness of available community resources, support services and educational programs through client engagement and collaboration with local service providers.
Interview clients and their families to assess the nature and extent of challenges affecting their wellbeing, including housing, financial hardship, family relationships, disability, and community participation.
Ensure all services are delivered in accordance with child protection legislation, disability standards, workplace health and safety requirements, privacy obligations and organisational policies while maintaining a client-centred, culturally respectful and ethical approach.
Support children, young people, older persons and individuals with disability by providing practical assistance, life skills development,



community access and participation in accordance with individual support plans.
Qualification & experience Relevant Study in the field Previous experience in a Family Support Worker, Community Support Worker, Case Worker or similar community services role for a minimum of 1 year.
Sound knowledge of person-centred practice, child and family support principles, and community service frameworks.
Ability to assess client needs, develop support plans and maintain accurate case documentation.
Strong communication, interpersonal and relationship-building skills with clients and stakeholders.
Experience working with vulnerable individuals, families, children and people with disability from diverse cultural backgrounds.
Knowledge of relevant legislation, including privacy, child protection, safeguarding and workplace health and safety requirements.
Proficiency in Microsoft Office and electronic client management systems.
Current National Police Check, Working with Children Check and NDIS Worker Screening Check (or ability to obtain).
Current
First Aid and CPR Certificate.
Current Australian Driver's Licence and reliable vehicle (preferred).
