By fostering a culture of respect, collaboration, and continuous improvement, Holicare is committed to not only raising the standard of care, but also building a workplace where employees feel valued, supported and inspired.As a Program Manager, you will:Build and maintain strong referral networks (GPs, hospitals, allied health, support coordinators and community organisations)Identify and pursue current referral opportunities within the local communitySupport growth targets for Support at Home and NDIS servicesConduct client intake,
home visits and assessmentsDevelop person-centered care plans aligned with funding guidelinesCoordinate and monitor service delivery to ensure quality and continuityMonitor client budgets and ensure services remain within funding guidelinesAct as the key contact for clients and familiesYou will play a key role in strengthening Holicare's community presence while ensuring high-quality, compliant and person-centered service delivery.About YouExperience in aged care, disability or community servicesStrong knowledge of Support at Home and/or NDIS funding modelsStrong case planning and case management skillsConfidence in building professional networks and promoting servicesExcellent communication and stakeholder engagement skillsAbility to manage KPIs and competing prioritiesCurrent driver's license and willingness to travel locallyExperience with systems (e.g. Nightingale, EziPlan, Share Point)Understanding of quality and safeguarding frameworksQualifications in Community Services, Nursing, Social Work Case Management, or related fields are highly regardedWhy Join Holicare?
