About the Role
Join the CO-ACT team at Hampstead Rehabilitation Centre and support older people to achieve the best possible outcomes through person-centred social work.
You'll manage complex caseloads, work closely with multidisciplinary teams and help patients transition safely to the next stage of their care.
Deliver complex psychosocial assessments and interventionsSupport safe discharge and care planningPartner with multidisciplinary teams and community servicesMentor junior staff and social work studentsDrive quality improvement and research initiatives
About You
You're an experienced Social Worker who is passionate about supporting older people through compassionate, evidence-based care.
Degree eligible for AASW full membershipExperience managing complex health care caseloads and discharge planningStrong assessment, counselling and case management skillsAbility to work collaboratively across multidisciplinary teamsExperience providing supervision and clinical guidance
