Primary objective

Work collaboratively with Outreach team, service providers and community members to plan, implement and evaluate health

promotion programs to increase capacity of Aboriginal people living in remote communities to improve their health and well-

being.

Key duties and responsibilities

1. Coordinate the planning, implementation and evaluation of evidence-based health promotion and community





development programs including the use of contemporary continuous quality improvement (CQI) tools and frameworks

in the development of health promotion programs within a multidisciplinary team environment.

1. Use community development principles work with community members/stakeholders to identify measurable health

promotion activities, events, goals and objectives based on their identified needs.

1. Provide education and training to Population & Primary Health Care staff around culturally appropriate resources for

delivery of health promotion to remote communities.

1. Work in collaboration with the Population & Primary Health Care workforce to support community based workers to

facilitate the planning, implementation and evaluation of Aboriginal Health promotion activities.

1. Provide strategic advice and contribute to the development and delivery of primary health care services, strategic health

promotion policies and workforce planning.

1. Follow defined service quality standards, work health and safety policies and procedures relating to the work being

undertaken in order to ensure high quality, secure services and workplaces.

Selection criteria

Essential

1. Demonstrated knowledge of planning,



implementing and evaluating of community-based health promotion and

community education programs and activities in remote Aboriginal communities

1. Experienced facilitation of group discussions, consultation and negotiation with Aboriginal people particularly in remote

communities pertaining to their health concerns and needs.

1. Demonstrated knowledge and experience in sharing health information to assist communities to clarify their own health

issues, prioritise health promotion activities, identify resources and build capacity for decision making action.

1. Knowledge and understanding of contemporary health promotion, Primary Health Care and community development

principles, theories and frameworks, with ability to build knowledge of the health and community workforce

1. Hold a current NT manual drivers licence or have the ability to readily obtain and have the ability to travel to remote

communities by 4WD or light aircraft for extended periods of time.

Desirable

1. Certificate IV or higher in Health Promotion, Population Primary Health Care
2. Certificate IV or higher in Workplace Training and Assessment
