BlueCare in Gympie and surrounding areas is seeking a Care Partner to ensure client-centred, high‐quality care by overseeing care planning and coordination with clinical teams. You will develop and review care plans in collaboration with clients and the multidisciplinary team, and facilitate budgets and services to achieve optimal outcomes.
This permanent full‐time role requires a current Driver's Licence, the ability to work on the road, and solid communication and problem‐solving skills.
