Are you passionate about supporting Aboriginal and Torres Strait Islander Elders to access culturally appropriate aged care services? Gidgee Healing is seeking a dedicated Elder Care Connector to join our Elder Care Support team in Mount Isa., As the Elder Care Connector, you will:
Conduct community outreach to promote aged care services.
Engage with Elders and families to raise awareness of aged care entitlements.
Support clients in identifying their goals and accessing appropriate services.
Assist with My Aged Care registration and review processes.
Advocate for clients and help identify service gaps in the region.
Work closely with the Aged Care Support Coordinator and internal teams.

About the Candidate
Certificate in Community Services or relevant experience.
Robust relationships with local Aboriginal and Torres Strait Islander communities.
Excellent communication and interpersonal skills.
Understanding of person-centred care and trauma-informed practice.
Experience working with older people and knowledge of aged care services.
Ability to work independently and as part of a team.
Additional Requirements
Current QLD C Class Driver's Licence.
Satisfactory police check and current Blue Card (or willingness to obtain).
Flexibility to travel and stay overnight in remote areas, including travel by road and small plane.
