About the Role Healthy Mothers Healthy Babies | Grade 2 Social Work | Part Time (0.6 EFT), Ongoing
As a Social Worker, you'll provide specialist psychosocial assessment, care planning, advocacy and support to women throughout pregnancy and into the early postnatal period. Working within a multidisciplinary team, you'll deliver evidence‑based, family‑centred care through a Social Model of Health, supporting women across hospital and community settings, including homes, outpatient clinics and other community locations. This is a rewarding prospect to build trusted relationships with families, address psychosocial risk factors, connect clients with community supports, and help improve health and wellbeing outcomes for both mothers and babies.

About the Program The Healthy Mothers, Healthy Babies (HMHB) Program is part of the Child and Family Program within Monash Health Community and Monash Children’s.
Our multidisciplinary team of Social Workers,
Community Development Workers and a Nurse‑Midwife works alongside women and families experiencing complex social, health and wellbeing challenges during pregnancy and the early postnatal period. We provide outreach‑based care that is strengths‑based, trauma‑informed and evidence‑based, recognising that healthy families are supported by addressing the social factors that influence health. Through advocacy, education, care coordination and practical support, we empower women to achieve the best possible outcomes for themselves and their babies.

About You
You're a compassionate and motivated Social Worker who is committed to delivering high‑quality, person‑centred care to women and families experiencing complex vulnerability. You bring strong clinical assessment skills, sound professional judgement and the ability to build trusted relationships with clients, colleagues and community partners. You enjoy working both independently and as part of a multidisciplinary team, and you're passionate about improving health and wellbeing outcomes through evidence‑based practice.
You will also bring: A commitment to safe, inclusive and family‑centred care. Excellent psychosocial assessment, communication and advocacy skills.
Experience supporting clients with complex psychosocial needs, ideally within maternity, child and family, paediatric or community health settings. The ability to work collaboratively across hospital and community environments. Strong organisational and documentation skills, with the ability to manage competing priorities.
A flexible, proactive approach and a genuine commitment to continuous learning and service improvement. Experience working with families from culturally and linguistically diverse communities.
