Omnia Community Health was established through the merger in January 2026 of Sunbury and Cobaw Community Health and Nexus Primary Health. Bringing together the deep community connections, shared values and specialist expertise of both organisations, Omnia builds on more than 50 years of experience delivering health and wellbeing services within the not-for-profit sector.

We offer a broad range of services across the lifespan including general practice, allied health, disability support, mental health, early childhood, family services, aged care and community wellbeing. These services are delivered from our sites in Broadford, Kinglake, Kyneton, Romsey, Seymour, Sunbury and Wallan, with additional outreach across surrounding communities.

About the Role

As part of our broader Health and Medical Services division, this role is a valuable addition to our Adult Allied Health unit, which comprises five multidisciplinary teams working collaboratively to support the health and wellbeing of our community.

The Team Leader Adult Allied Health (North 2) provides both clinical and operational leadership to a multidisciplinary team delivering high-quality, community-based allied health services. The role oversees service planning, delivery and evaluation to ensure high-quality,



person-centred care.

Working collaboratively with staff, clients, and key stakeholders, the Team Leader will foster a culture of excellence, support the delivery of person-centred care, drive continuous service improvement initiatives, and promote a positive, engaged, and high-performing team environment.

The position is responsible for staff supervision, performance management, caseload allocation and workforce development, while also supporting recruitment and onboarding activities.

The Team Leader manages budgets and resources, monitors service demand and quality outcomes, supports accreditation and compliance requirements, and contributes to organisational priorities.

About You

You are someone who:

- Can demonstrate leadership, supervision and performance management experience. With an ability to work collaboratively within multidisciplinary teams, foster a positive team culture and contribute to high-quality client outcomes and service improvement.
- Demonstrates a commitment to the principles of community health and the social model of health.
- Has knowledge of funding streams including but not limited to NDIS and Aged Care reforms.
- Has the ability to analyse data to support informed decision making and the ability to plan, evaluate and improve service delivery.




- Has high level written and verbal communication skills along with strong organisational and time management skills.
- Holds a tertiary qualification in occupational therapy, physiotherapy, exercise physiology (preferred) or another relevant allied health discipline.
- Has a current registration or membership with the relevant professional body.

The successful candidate will berequired to undertake and maintain a National Criminal History Check, NDIS Safety Screening check, WorkingwithChildren Check and hold valid Australian working rights.
