Ambition Health Group is a growing care organisation supporting people across disability, community nursing, supported accommodation and Support Coordination services.

We are committed to delivering reliable, person-centred support that helps participants achieve greater independence, choice and control. As our services continue to grow, we are looking for an experienced and motivated Support Coordinator to join our team.

About the Role

The Support Coordinator will work closely with NDIS participants, families, nominees, service providers and allied health professionals to help participants understand and implement their NDIS plans.

You will support participants to identify suitable services, build their capacity, manage risks and work toward their individual goals. This role requires strong communication, accurate documentation, initiative and the ability to manage a participant caseload effectively.

Key Responsibilities

- Assist participants to understand and implement their NDIS plans.
- Connect participants with suitable mainstream, community, funded and informal supports.
- Build participants’ capacity to manage their supports and make informed decisions.
- Develop strong working relationships with participants, families, nominees and providers.
- Coordinate services across allied health, accommodation, community support and other relevant areas.
- Monitor participant budgets and help ensure supports remain within available funding.
- Identify service gaps, risks and barriers and elevate concerns appropriately.
- Support participants through plan reassessments,



change-of-circumstance requests and funding reviews.
- Prepare progress reports, case notes, service agreements and supporting documentation.
- Maintain accurate and timely participant records in Brevity.
- Respond promptly to participant enquiries and maintain regular communication.
- Attend stakeholder meetings, case conferences and participant reviews.
- Manage incidents, complaints and safeguarding concerns in line with organisational procedures.
- Meet agreed performance, documentation and billable-hour expectations.
- Work collaboratively with internal departments and external stakeholders.

About You

We are looking for someone who is organised, confident, compassionate and able to manage complex situations professionally.

The successful candidate will have:

- Previous experience in NDIS Support Coordination or a similar role.
- A strong understanding of the NDIS and participant plans.
- Knowledge of local disability, health, housing and community services.
- Strong case-management and stakeholder-engagement skills.
- Excellent written and verbal communication.
- Strong time-management and organisational skills.
- The ability to manage competing priorities and work independently.




- Confidence working with participants who have complex support needs.
- Strong documentation and report-writing skills.
- The ability to maintain professional boundaries and confidentiality.
- Competence using client-management systems, email and Microsoft Office.

Essential Requirements

- Relevant qualification in community services, disability, social work, psychology, mental health or a related field.
- Current NDIS Worker Screening Check.
- Current Working with Children Check, where applicable.
- Current National Police Check.
- Valid Australian driver’s licence.
- Reliable vehicle and willingness to travel when required.
- Current First Aid and CPR certification.
- Evidence of the right to work in Australia.

Desirable

- At least two years of Support Coordination experience.
- Experience supporting participants with complex needs.
- Experience with SIL, SDA, psychosocial disability or complex clinical supports.
- Understanding of NDIS funding categories and plan-management arrangements.
- Experience preparing reports for NDIS plan reviews.
- Existing relationships across the NDIS and community-services sector.
