Support Worker - SIL, In-Home Care & Community Participation

Rainbow Palm seeks to empower participants to live life as independently as possible and offers a variety of attendant care supports to help them achieve their goals. We aim to provide premium supported services to permanent and respite residents within a smaller setting accommodation, such as Supported Independent Living, Specialist disability accommodation, through National Disability Insurance Scheme.

POSITION DESCRIPTION

We are seeking a dedicated, compassionate, and reliable Disability Support Worker to join our growing team. This multifaceted role combines high-quality personal care with active community engagement. You will support participants in maintaining their health, well-being, and independence within a residential (SIL/SDA) setting, while facilitating meaningful social and community participation aligned with their individual NDIS goals.

DUTIES & RESPONSIBILITIES

- Medication Assistance: Administer and prompt medication accurately in strict accordance with the participant’s medical plans and organisational policies.
- Personal Care: Assist with personal hygiene tasks—including grooming, bathing, dressing, and toileting—while promoting independence and preserving participant dignity.
- Health & Mobility: Support participants with physical therapy exercises, rehabilitation treatments, and the safe use of mobility aids (e.g., hoists, slide sheets, wheelchairs).
- Nutritional Support: Assist with meal planning, preparation, cleanup, and the delivery of fluids or modified diets, ensuring nutritional plans are followed.
- Clinical Support: Assist visiting nursing or allied health staff with specified, non-complex treatments within the scope of a support worker role.

2. Community & Social Engagement

- Goal Implementation: Plan and facilitate social, recreational, and capacity-building activities tailored to individual interests, choice, and NDIS goals.
- Community Outings:



Accompany and transport participants to community events, recreational activities, medical appointments, and personal outings.
- Social Connection: Foster meaningful connections to prevent isolation and loneliness, always respecting the participant's choice to engage.

3. Positive Behaviour Support & Safety

- Behaviour Support: Implement Positive Behaviour Support Plans (PBSP) and manage behaviours of concern safely and empathetically.
- Restrictive Practices Compliance: Ensure all crisis interventions strictly adhere to the guidelines set by the NDIS Quality and Safeguards Commission, completely avoiding unauthorised restrictive practices.
- Incident & Emergency Response: Respond quickly, calmly, and responsibly to emergencies or medical crises by following established first aid and emergency procedures.

4. Compliance, Documentation & Communication

- Aids & Communication: Support participant communication effectively, including the fitting and maintenance of hearing aids, communication devices, and glasses.
- Record Keeping: Maintain accurate, objective, and timely records of participant health data, activity participation, shift notes, and progress toward goals.
- Reporting: Observe, monitor, and report changes in a participant’s physical, mental, or emotional well-being to senior management immediately.
- Collaborative Practice: Work collaboratively with internal team members, families, and external allied health agencies to deliver seamless, coordinated support.
- Operational Support: Perform necessary household tasks—including general cleaning, hygiene sanitisation, laundry, and property maintenance—to maintain a secure, welcoming home environment.
- Flexible Support: Provide high-intensity 1:1 Community Participation (CP) or complex support as operationally required by the business.

SKILLS & EXPERIENCE

Mandatory Qualifications & Compliance Checks

- Dementia Specialisation:



Proven experience and specialized competency in supporting individuals living with dementia, cognitive decline, or memory loss.
- Industry Experience: Minimum of 2 years of experience working within disability support, complex personal care, or community-based settings.
- Education: Certificate III in Individual Support (Disability), Certificate IV in Disability, or a relevant health/community services qualification (or equivalent demonstrated industry experience).
- Worker Screening: Valid and current NDIS Worker Screening Check clearance.
- Child Safety: Current Working with Children Check (WWCC).
- Criminal History: Current National Police Check with no disclosable outcomes.
- First Aid: Current First Aid Certificate and CPR certification.
- NDIS Training: Completion of the NDIS Worker Orientation Module.
- Infection Control: Valid Infection Control training certification.
- Driver’s Licence: Valid Victorian Driver’s License and access to a roadworthy, fully insured vehicle.
