The Role
Home-Focused Care Planning: Lead client intake, comprehensive in-home assessments, reassessments, budgets, and individualised care plans that reflect each client's home environment and personal goals.
Support Worker Mentorship: Induct, supervise, and develop Community Care Workers with practical guidance on delivering safe, person-centred support in clients' homes.
Hands-On Clinical Care in the Community: Manage wound care, medication administration, catheter care, and other complex treatments within the home setting.
Continuity of Care: Act as the key link between clients, families, hospitals, GPs, and allied health professionals to ensure seamless transitions and coordinated support at home.
Quality & Compliance: Drive continuous quality improvement initiatives and ensure alignment with Aged Care Quality Standards, NDIS, and My Aged Care—always with a focus on enabling independence at home.
What You'll Bring to the Team
Proven Community/In-Home Experience: You have worked in home care, community nursing, or case management supporting clients to remain safely in their own homes.
You understand falls prevention, manual handling in home environments, and working with families and informal carers.
Solid Clinical Foundation: Quality-first mindset with sound nursing judgment, particularly in wound care, medication management, and chronic disease support.
Exceptional Communication: You can build trust with clients and families in their own homes and communicate clearly with multidisciplinary teams.
Tech Literacy: Proficient in Microsoft Office and Excel for care planning and reporting.
A valid Australian Driver's Licence and access to your own reliable vehicle (essential for community visits).
Full Australian working rights and a valid Police Check.
