With a focus on improved client outcomes, the role will collaborate closely with the wider Clinical team and will be involved in delivering individual care plans.
The role will provide supportive, respectful and quality personal care that aligns with the Aged Care standards of practice.Key responsibilities:Provide home based care and support services including personal care, domestic assistance medication prompting and other activities of daily living.Implement the individual care plan developed by our Nursing teamAdhere to and support best practice in infection control, waste management and universal precautionsAssisting clients with mobility,
