Located in Dee Why, we offer Inpatient and Outpatient Rehabilitation for acute medical and surgical patients.
This includes orthopaedic, reconditioning, neurological rehab as well as cardiac rehabilitation.
Delmar Private Hospital boasts spacious private rooms with ensuite, three well equipped gyms, two hydrotherapy pools, as well as free in office parking.
Delmar Private Hospital is seeking a Permanent Part-Time Social Worker (22.5 hours per fortnight) to join our team.
We are looking for someone with experience in rehabilitation and a passion for high-quality patient focused care.
You will work within a skilled and supportive multidisciplinary team with a strong focus on excellence in care.
Our Social Worker is an integral part of our multi-disciplinary team.
Working closely alongside our Physiotherapists, Exercise Physiologists, Dietitians, Occupational Therapists, Clinical Services Manager, Rehab Co-ordinator and Discharge Planners.
About the Role:
Work within a skilled and supportive multidisciplinary team
Participate in client specific goal setting and case conferences
Provide vital input to patients, their families and carers to provide ongoing support and care following their stay in inpatient rehab.
Responsibilities:
Providing comprehensive Social Work assessment and support for our inpatient's cohort,



including case mixof Orthopaedic, Neurological, Cardiorespiratory and general reconditioning
Facilitate the referrals to My Aged Care, for increase or commencement of services, including TRANSPACs, Home Care and RCP's.
Work with patients to develop client specific goalsto facilitate safe and timely discharge plans or social integration
Participate in weekly multi-disciplinary case conferences
Work with the team to improve service delivery and processes
Participate in and contribute to internal professional development and in-service program
Qualifications & Experience:
Degree in Social Working, with AASW Registration
Experience in a rehabilitation setting, or with Aged Care services
Experience working as part of an MDT
Current National Police Check and working with children clearance
Proven ability to work unsupervised and exercise independent professional judgement
Demonstrate a high level of interpersonal, written and verbal communication skills
Demonstrated time management and organisational skills
Demonstrated application of sound clinical reasoning skills
Knowledge of equipment provision requirements, as well as understating of My Aged Care and community services, including TRANSPAC, RCP's
