Synergie Australia is seeking multiple APS6 Clinical Care Coordinators for a trusted Federal Government client located in Surry Hills with further roles also available across Australia.
The important parts:
Pay: $*****–$***** per hour + Super | Duration: 24-month contract
Hours: Monday–Friday (37.5 hours per week)
Location: Surry Hills, NSW

What you will be required to do:
As a Clinical Care Coordinator, you will play a critical role in supporting veterans and their families to access and engage with mental health services, addressing barriers and improving overall wellbeing.
You will:
Provide short-term, goal-focused clinical case management
Support clients with complex psychosocial needs and barriers to care
Conduct risk assessments, care planning and brief interventions
Coordinate referrals and connect clients with appropriate internal and external services
Build strong relationships with community providers, DVA and support services
Work collaboratively within a multidisciplinary clinical team




About You
You bring strong interpersonal skills, empathy, and the ability to engage with diverse stakeholders.
You thrive in community-facing roles and are passionate about making a genuine difference.
Experience in community services, case management, veteran support, or government programs will be highly regarded.

To be successful in the role you will:
Experience in mental health, case management or psychosocial support
Strong ability to manage complex or high-risk clients
Confident in risk assessment, care coordination and stakeholder engagement Excellent communication skills and emotional intelligence
Relevant tertiary qualification in: Psychology, OT or Nursing (AHPRA), OR Social Work (AASW), OR Counselling (PACFA/ACA)
Working With Children and/or Vulnerable People Check (or ability to obtain)
Ability to obtain and maintain a Baseline Security Clearance + Criminal History Check
Experience in case management, risk assessment and complex client support
