You will be working closely with the Care Coordinator and assisting in the development of care plans for clients based on their individual needs and help to facilitate the delivery of care and services for clients in home care.Key ResponsibilitiesAssist the Care and Services Coordinator to develop care plans and provide the highest quality of client care and supportCover weekend shifts on a roster basis and working effectively under minimal supervision at these timesAssist in facilitation of appropriate and agreed care and services within client budget constraintsMaintain staff rosters and ensure client coverage after hours when required,
