Case Manager – Intensive Therapeutic Care – Significant Disabilities (ITC-SD)Wesley Mission is seeking a passionate and experienced Case Manager to join our Intensive Therapeutic Care – Significant Disability (ITC-SD) team.

We support people experiencing disadvantage through disability services, aged care, homelessness support, child and family programs, counselling, employment, education and health.Our Vision is A Spirit-led disciple-making movement: doing all the good we can because every life matters.About the ProgramThe ITC-SD program provides therapeutic residential care to children and young people diagnosed with a significant disability in statutory Out-of-Home Care.
Our team delivers trauma-informed, person-centered support with a robust focus on permanency, recovery and life-skill development.About the roleAs Case Manager, you will deliver high-quality case work services to children and young people in Intensive Therapeutic Care.
You will lead permanency planning, develop and review case plans, and work collaboratively with therapeutic specialists, house managers and support workers.This role is fast-paced, rewarding and deeply impactful - ideal for someone who thrives in a multidisciplinary environment and is committed to child-centered practice.Key responsibilities include:Developing, implementing and reviewing therapeutic case plansFacilitating case reviews and completing all required documentationSupporting identity development,
life story work and life skillsBuilding strong partnerships with Aboriginal, CALD, specialist and mainstream servicesEnsuring compliance with OCG Child Safe Standards and DCJ OOHC Case Management PolicyMaintaining accurate records and contributing to continuous improvementQualificationsBachelor degree in Social Work, Psychology or Social WelfareExperience supporting young people with complex care needsStrong understanding of OOHC policy, standards and quality frameworksExcellent communication, assessment and planning skillsAbility to work independently and within a multidisciplinary teamWorking With Children Check and satisfactory criminal history checkRemunerationSalary in accordance with SCHADS Award ****, Level 5Fully maintained vehicle, including fuel, for professional and private useGenerous salary packaging optionsOngoing training, supervision and professional developmentSupportive team culture and opportunities for career advancementApplications close Friday 14 August **** at 5pm.Additional InformationWesley Mission is a Christian organisation requiring all staff to affirm its values and Worker code of conduct.
