Pregnancy Family Conferencing (PFC) uses family conferencing with an independent facilitator to promote early engagement and interagency planning with pregnant women and families at risk of their newborns entering out of home care at birth.
Early engagement and partnerships between Health and the Department of Communities and Justice aims to reduce risks and the number of infants assumed into care and/or the identification of an appropriate carer prior to birth.
The program utilises a strengths-based model which promotes a participatory and transparentprocess for parents and families.
The coordinator is also responsible for supporting drugs/substance use in pregnancy program outcomes.
The coordinator is responsible for the overall leadership and management of the program and undertaking clinical work including to:
Coordinate, allocate and oversee all PFC program cases
Develop and maintain a group of appropriately skilled facilitators
Undertake the facilitation role, as required
Develop and foster strong interagency partnerships to sustain and further develop the program
Ongoing development, implementation and evaluation of the program
