Duties
Effectively engage clients(extended family and kin of child)through cold-call home-based visits. Turn initial 'kin engagement'home visitintoongoingconsentand interest in becoming a Kin Carer.
While completingfamilyengagementactivities,ask toidentifyotherstrong family members that couldpossibly becomea kin carer. Actively seekcontactdetails and relationship connection with child and broader family.
Engage the child and/or lead Residential Care Worker indeterminingthe views andwishes of family placement options with family, asdirected by line Manager. Utilise the Narrative Assessment Tools to assessbarriers and wishes.
Assistin the assessment of carers, including initial screening, Blue Card submission, and PAC assessments.Advocate and support families when being assessed externally.




Develop a comprehensive Transition Plan(Case Plan)that explicitly addresses allconcernsoutlined in the (1) referral, (2) program assessment tool, (3)TransitionNAT tools, (4) Child Assessment,and(5) Carer Assessment.
Deliver case managed services according to the program model of care, ensuringminimum case load requirements aremaintainedto ensureequitableaccess to services for community.
Maintainaccurateand detailed case notes of all client interactions, ensuring case notes are written from the perspective of clients in their voice.
Ensure clients completethe 'Have Your Say' client feedback tool andsubmit. Act on the client feedback to make continuous improvements to your practice and approach with clients.
As perMOBnetPerformance module,participatein monthly prescheduledsupervision with direct lineManager.Ensure Supervision tasks are thepriorityin work completion.Ensure all associated monthly supervision tasks inMOBnetarecompleted (two-way feedback, team culture scoreetc).

Base Salary of $105,673per annum (Based on qualification and experience).
12% Super + access to salary packaging (Increases your take home pay).
Three Permanent Full-Time positions.

Required
Degree Qualification+Moderate Experience:Bachelor's degreein a relevant social services field withat least 4 yearscase managementexperiencein family support, child protection,



youth support,housingor related field.
OR
Diploma+High Experience:Diplomain relevant Community/Services fieldwithat least 6 years in a case management rolein family support, child protection, youth support,housingor related field.
OR
Nil Qualifications+ High Experience +StrongCultural Engagement:At least6 years case managementexperiencein family support, child protection, youth,housingor related field. Acommitment tocomplete Diploma(funded by ATSICHS) while working.Andidentify asAboriginal and/or Torres Strait Islander(with confirmationproof)ORnon-indigenous and having worked for an Aboriginal Community Controlled Organisation forat least 4 years.
And
Exceptional case management skills, including grammatical writing ability, personal organisation and time management, and execution of services according to the model of care phases of work.
Demonstrated ability to create current social work theoretical frameworks of practice, aligned with ATSICHS values. Ability to coach other staff on client interventions that build client motivation, insight and acceptance, self-awareness, decision making (agency), and sustainable behaviour change.
Ability to apply technical knowledge of relevant legislation (child protection/housing/youth justice etc). Ability to critique/ and problem solve legislative grey zones, while managing organisational and child/client harm risks.
Influence decisions of external stakeholders, including Child Safety, Youth Justice, Schools, and Hospitals. Whole-of-system Internal Integration: Ability to connect through systems panel coordination numerous internal ATSICHS programs and services to activate wrap-around care for clients, trading-off and negotiating resources across the organisation and between other teams to achieve client impact.
Additional Requirements
Working with Children's Blue Card or ability to apply.
National Police Certificate.
Driver's License.
Evidence of Work Rights.
Travel to work in the community on a regular basis.
Able to work after-hours and on weekends.
