Gladstone Community Linking Agency is seeking an experienced Aged Care Care Partner (CHSP) to join our team in Gladstone, QLD. This part time (30 hours per week) role is a 12-month maximum-term contract designed to support older Australians to live independently at home.

You'll manage a caseload, develop care plans, coordinate services, and monitor wellbeing to meet funding requirements and the Aged Care Quality Standards.
