Your Role
As a Clinical Care Coordinator,



you'll take a hands-on leadership role in clinical care delivery while supporting and mentoring the nursing and support teams.
Manage care planning and assessments
Lead a team of RN's, EN's and Support staff
Managing complex wound care and coordinating allied health referrals
Oversee quality and carry out education for clinical staff.
Partner with families and allied health teams to deliver holistic care
Foster a safe, supportive, and resident-focused workplace culture
About You
Bachelor of Nursing with current AHPRA registration
1-2 years experience working as a Senior Nurse/Clinical Coordinator
Proven ability to provide strong clinical leadership, mentoring, and support to nursing and care teams
Sound knowledge of accreditation standards, compliance requirements, and care planning processes
A genuine commitment to delivering high-quality, resident-centred care
