Tea Cup Cottage is seeking compassionate Support Workers to join our Transitional Care team in a residential setting. You will work under the guidance of Registered Nurses to support participants moving from acute hospital care into a secure home workplace.

Responsibilities include personal care, mobility, medication support, documentation, and cooperative care planning. A values-driven, team-focused approach helps us deliver high-quality, person-centred support.
